گزارش یک مورد انسولینوما

چکیده انسولینوما شایعترین تومور سلول جزایر لانگرهانس است و اغلب در محل تنه و دم پانکراس می‌باشد. این تومور، در بیشتر موارد اسپورادیک می‌باشد؛ اما ممکن است همراه با سندرم تومورهای متعدد اندوکرین نوع 1 (MEN1) نیز مشاهده شود. در بیشتر از 90% موارد خوش‌خیم است. تعیین محل قبل از عمل به وسیله رادیوگرافی ممکن است مشکل باشد. لمس حین عمل و اولتراسونوگرافی روش استاندارد طلایی برای تشخیص می‌باشد. بیمار گزارش‌شده مرد 27 ساله‌ای با سابقه حملات مکرّر، ضعف و تعریق، سردرد، کاهش هوشیاری و افزایش وزن می‌باشد. قند خون ناشتای وی mg/dL 40 و نسبت انسولین به گلوکز بین 57/0تا 76/0 بود. سی‌تی‌اسکن شکم توموری را در پانکراس نشان نداد. بیمار لاپاراتومی شد و توموری در ناحیه سر پانکراس لمس گردید که برداشته شد. طبق یافته‌های بالینی و ریزبینی، برای آن تشخیص انسولینوما مطرح گردید. هدف از گزارش این مورد آشنایی بیشتر با این تومور نادر و روش رسیدن به تشخیص با توجه به روشهای آزمایشگاهی می‌باشد.
